Spinal Deformity Care
PART I
Information items
01Institutional capabilities and essential infrastructure
Pediatric and adult spine orthopedics/neurosurgery, pediatric-adolescent neurology/rehabilitation/imaging/anesthesia/critical care/respiratory/bone metabolism collaboration is required. Pre-standing full-spine AP/lateral and side-bending images, low-dose whole-body alignment imaging, CT/MRI, growth maturity/pulmonary function/gait/bone density assessment, customized orthoses, scoliosis-specific exercise, 3D navigation, intraoperative neuro-monitoring, cell saver, ICU response, and multi-segment fixation, osteotomy, and growth-preserving surgery capabilities are required.
02System capabilities and collaborative-care structure
After performing a forward bending test and neurological examination, evaluate Cobb angle, curve type and flexibility, Risser/Sanders growth maturity, and progression rate. For children, stage the treatment as observation–exercise–bracing–growth-preserving/corrective surgery, and for adults, set treatment goals considering global upright alignment, source of pain, nerve compression, hip/knee compensation, bone quality, and frailty. After surgery, perform long-term follow-up on neurological function, respiration, infection, alignment, junctions, fusion, function, and patient-reported outcomes.
03Recognition unit
Scoliosis center, spinal deformity clinic, or dedicated pediatric-to-adult deformity correction team are recognized. General disc or stenosis treatment alone is insufficient. During growth, conservative treatment, pediatric/neuromuscular deformities, adult global alignment assessment, complex corrective surgery, and core elements in nerve monitoring, intensive care, and rehabilitation must be confirmed with official evidence.
04Evidence and verification criteria
Prioritize diagnostic, conservative treatment, adult deformity, and safety data from the Scoliosis Research Society (SRS), SOSORT, and AO Spine, and verify performance capabilities using hospital official centers, medical staff, and outcome data. Institutional evaluation includes patient composition by age/cause, brace compliance·curve progression, surgical volume, neurological injury·infection·reoperation·junction failure, correction maintenance, SRS-22·ODI·gait·quality of life.
05Currency status

Detailed information item
01Definition of specialist medical care and domestic and international clinical statistics (Definition & Epidemiological Context)
Spinal deformity is a three-dimensional disease in which not only the scoliosis seen from the front and back but also spinal rotation, sagittal curves such as kyphosis and lordosis, and the balance of the head and pelvis change together. The SRS defines scoliosis as a Cobb angle of 10 degrees or more, and during growth, treatment is chosen from observation, exercise, brace, or surgery depending on remaining growth and progression rate. In adults, conservative treatment is generally prioritized, and corrective surgery is considered when persistent functional impairment, neurological symptoms, imbalance, and decreased quality of life are significant.
02Patient problem and clinical entry point (Patient Problem Layer)
During growth, shoulder·scapula·waistline·pelvic height asymmetry, one side of the back protruding during forward bending, rapid curve progression requires specialist evaluation. Night pain, neurological abnormalities, atypical curves, very early onset may require evaluation for causes such as MRI. In adults, forward bending or tilting to one side, limitations in standing·walking, neurological leg pain·muscle weakness, and compensatory hip·knee bending are observed together.
03Precision Diagnostics and Functional Evaluation System (Diagnostics & Functional Evaluation)
Measure Cobb angle, coronal balance, thoracic kyphosis/lumbar lordosis, SVA, and PI–LL, PT, SS on standing AP/lateral spinal images, and check flexibility on lateral bending images. In children, assess Risser/Sanders stage, menarche, growth rate, congenital anomalies, neuromuscular disease, and lung function. In adults, use MRI/CT to assess stenosis, rotation, fusion status, DXA/CT for bone quality, and set frailty, nutrition, cardiopulmonary function, gait, and SRS-22/ODI as baseline.
04Treatment Methods and Timeline-based Care Pathway
Step 1: Confirm structural deformation and risk of progression → distinguish postural asymmetry and structural deformation, and evaluate growth, cause, curve, neurological risk.
Phase 2: Conservative treatment and regular follow-up → Observe low-risk cases; for growing spines at risk of progression, implement scoliosis-specific exercises, customized braces, and compliance management.
Stage 3: Customized corrective surgery → early onset is for growth preservation, progressive adolescents undergo safe 3D correction and fusion, and adults choose decompression, osteotomy, and segmental reconstruction.
Stage 4: Growth·alignment·functional long-term management → neurological·respiratory·infection·junction·fusion·curve/alignment maintenance and rehabilitation·quality of life are tracked.
